It is not a form, but a letter from your parent giving their consent to their money being used in your application. Without this consent letter, you will get no points for maintenance. What do you mean by "sponsor"? You can only show evidence of money in your own name or in a parent's name, unless you have funding from an official financial sponsor but that would not be an individual anyway.

For undergraduate students, the uni considers it to be period between academic terms. Postgraduate students, including PhD, are not entitled to vacation during the normal university 'vacation' period. Tell him he needs to speak to his new Tier 4 sponsor if he has any questions about his Tier 4 application That is their role to guide him through his application, if he has any questions or concerns. It is really not something to outsource to random strangers online.
