The address on your licence doesn't matter.

What matters is that you declare where you've resided. The fact you've not updated DVLA is not an issue.

You'll be travelling on your Australian passport, and even though your UK visa will expire a short time before your return, you're still eligible for a visa on arrival for tourism in the UK as an Aussie. So, as long as you have return flights and you have enough time left on your Australian passport...

Spike2009, using the wrong form will mean your fee is forfeited. So, you're right to figure out which form is required before submitting it.
