Docs required to apply for T2 Gen from Canada

Hi Experts,

I would be applying for a T2G visa from Canada, request you to confim if the below docs are sufficient or I would need anything else as well.

1) T2G Main applicant + 2 Dependants forms filled
2) Bank statements + Funds maintained for 90+ days
3) Original degree certificate
4) Original marks card
5) NARIC certificate for English Language Assessment
6) NARIC certificate for Statement of Comparability
7) Photos (UK standards)
Old & new passports
9) CoS copy (the coS# would have been filled in the application though)
10) Marriage certificate

One copy with original certificates and once copy with photocopies.

On point no 2, what is the actual format which is expected by the UK embassy.
As in, what all details have to be present in the bank letter head.

Request you to advice.

No, I'm in Toronto, Canada.
I had to return back to India in June-2013, however I travelled to Canada on another assignment.
Staying in Canada, I was enquiring about my previous gaps to check if ILR is possible. But looks like now the gap is high and I have to go thru the Tier 2 General process to get an ILR and that will only be after 5 years from the time I enter.
