The permanent residence applications is something they are more cautious with, as once granted your rights are not dependent on the UE citizen anymore and they can not easily take it away from you. This is why they will take any option they have to decline it, for exemple if there is the slightest gap of employment/residence of the EU citizen in the past 5 years they will say you were not exercising treaty rights for the full duration (maybe this is what happened in your case?).
