NHS Fees for Child Born in UK

Hello Again,

I am On Tier 2 General since Aug 2014.
My child born in UK on Jan 2015.
Tier 2 Dependent visa has not been processed for my child yet.

As per the new NHS fee for Tier 2 General,
If i process Dependent visa for the child now, will i have to pay the NHS fees as well on top of the visa fees ?

Also, for a child born in UK, visa is required only at the time when the child travels outside and reenters UK right ?

1 more question:-

When i process the dependent visa for my child, do i have to opt only for a 5 year visa or can i opt for a 3 year visa ?
