mn1 form payment sheet?

Hi everyone,

Just wanted to confirm, the payment sheet we need to submit with MN1 form is "Master_fees_leaflet_2015" ??

or is it any other sheet?

Yes, the last sheet in the link is all you need to complete. There isn't anything else for payment.
