PBS dependant to Tier 2 (General)

Hi,
My partner is undergoing a movement from Tier 2 ICT Dependent Visa to Tier 2 General Visa.
She was working on Tier 2 ICT Dependent and has decided to join another firm which has sponsored her Tier 2 General. But the new employer is stating that they cannot issue a final offer letter until she gets a visa stamped. But they have given a CoS and a conditional offer letter (condition = she gets a visa).

1. Would not having a final offer letter create any issue for her during the visa process while she makes an out of country visa application?
2. We visited our home country for two weeks vacation in Jan'15 and then returned UK, does that need to be declared as a two separate entries in the section Travel History Section?
3. While she makes out of country visa application, what does she reply to "Does your spouse / partner currently live with you", as I would keep staying in UK?
4. For the Medical Treatment section, what does she reply? She had a few minor visits to NHS for flu/cold/dental and travel vaccinations done while she was on Tier 2 ICT Dependent Visa.
5. Must she detail about her current job in the "Additional Information" section? What else must be part of this section?
6. Documents - Apart from regular docs, which all docs must she present?
6a. Sal Slips from current employer?
6b. UK Bank Slips
6c. Council Tax Bills
6d. Utility Bills
