To Complete Visa Application for Tier 2 General

To Complete Visa Application for Tier 2 General

I plan to submit my Tier 2 General application soon. It would be really kind of you to help me with following while completing my application

Main address and contact details?
Can it be my work address and details of HR rep as I have not sorted my accommodation in the UK.


Have you made an application to the Home Office to remain in the UK in the last 10 years?
I have only stayed on student visa for 4 years (2008-2012). What do I choose? Yes/No?


Have you previously had any medical treatment in the UK? Did you have to pay for the treatment? Treatment start and end date?
As already mentioned, I stayed for four years as student and was registered with nearby medical practice. I did visit GP sometimes but mostly for minor issues and only once had to go for a ear test at one of the local hospitals (recommended by GP). I can give contact details of medical practice but what do I put in for treatment start and end date?


I have a PhD degree from a leading UK university, it does appear on the points calculator as well. I assume I am only required to submit original degree along with application and that shall be good enough for the language requirement points?


My sponsor has certified maintenance support but would it be useful to append my bank statements with the application as well (I have had more than required amount for over three months).


The space for Additional Information is limited, Can I attach a separate sheet for additional information, as required?

Should be fine and then explain in the 'additional information using the numbering of the form.

You can put general GP appointments for minor issues as required while on student visa. Specific dates/details not on hand.

Yes and make sure it is stapled to the form.

Dear CR001,
You are a star! Thank you for such prompt and helpful response.

Please clarify couple of things for me

I had got four year student visa from my home country so I never applied for a UK visa being in the UK. Do I still answer yes for "Have you made an application to the Home Office to remain in the UK in the last 10 years?"

Last thing, would it be necessary to get in touch with the medical practice and get detailed medical history to go with the application?
Though I am not sure if they would provide it soon enough and more importantly, would it be critical for the application?

1. You can declare you student visa. If you don't, they will pick up on it. They have more details on you than you think.

2. No, it costs £50 to get a copy of your medical notes and it can take weeks. Your medical notes are private, HO does not need to see them.
