Tier 2 dependant

Hi,
My current leave to remain was curtailed as my employer's licence was cancelled. I am in the process of moving on to Tier 2 dependent visa. I have a couple of questions,
1) Can I register a limited company and pay myself while the application process is ongoing?
2) Which form should I use for the incountry tier 2 dependent visa application?
3) I have already paid NHS surcharges till the end of next eyear, do I have to pay again? How do I claim the additional amount paid?
4) Is there any way to know what dates are available for inperson appointment without paying the money?

It will be really helpful and I will be really thankful if someone can guide on the above.

1. No, you can only do it once you have been granted a PBS Dependent visa.

2. It is an online form only, see here (click)

3. IHS has to be paid for each application you make. So yes, you will have to pay again and possibly request a refund of the previous part amount.

4. Yes, register and look online at availability. You only pay when you choose a date and book.

Hi Sorry for not being clear at first. I understand about the online form but when I try to book a premium appointment on this link (https://apply.ukba.homeoffice.gov.uk/iapply.portal) there is a dropdown with list of forms to select. This dropdown doesn't have any reference for Tier 2 Dependent related form for appointment booking.
