Form AN - Tax Ref Number & Points on License Question

Hi guys...

Need advise in following please

1) Is it mandatory to fill Address of tax office or tax office ref No? ( 1.49 in AN Form) Is this Number remain same/ unique even if one do/ has two different job/ work?

2) Had Three penalty points on licence while driving in 2010 ( CU80-Breach of requirements as to control of the vehicle, mobile telephone etc ). Except this never had any fine or tickets. ( Presently I endorsed my licence as it's been 4 years and now I have new licence without any points )
Accepted spot fine and penalty points as Police officer stopped me. Not convicted in Court
I never mentioned in my Tire-1 extension and with ILR as it not part of Criminal record as per my understanding.

Do I need to mention in my Naturalisation Application? If Yes How to Frame sentence.

It is the tax office or tax ref number for your CURRENT or MAIN employment. It is on your P60 or your HR department can give you the information. If you are unemployed, leave it blank.
