Tier 1 general switching to ILR - Dependent procedure

Hi Guys,
Can anyone help me with couple of queries below. I have Tier 1 (General Visa) and it is expiring on 22 January 2015. Therefore, I am applying for ILR on January 1st week in 2016.

My wife came to UK as a Tier 1 dependent on 22/Oct/2012 with short visa (22/Oct/2012 - 24//Jan/2013).
After that I have got Tier 1 General extension from 24/01/2013 - 25/01/2016. At the same time she got dependent visa (24/01/2013 - 25/01/2016).

1) I know she is not eligible for ILR dependent at the moment according to 5 years rule. But i would like to apply for dependent visa with me using same day service(premium)? For that I am confusing which form i need to use? What is the fee for premium?

2) I think i can apply for her dependent visa once i will get ILR. ( Because i am using same day service). For instance what would happen if my application will delay with home office. Can she still apply dependent visa if my application is pending with Home office. If that is the case what is the procedure for that one?

1. Dependants applying in the UK on their own.
2. If principal migrant's application is put on hold for some reason, the dependant application will also be put on hold - pending outcome of principal migrant's application.

I am in a similar situation, can you please advise me which form to fill?

Dependants applying in the UK on their own - Will be if the main applicatt is applying for Tier 1,2 or 4 not if the main applicant is applying for ILR.

The dependent is applying for an extension on their own (i.e. without you, the main applicant) if you have got ILR.
