ILR After Discretionary :NHS Surcharge

I am about to complete 6 years of discretionary leave which expires on 4 July and has held full time jobs for years. I would like to use the premium service "Do I have To Pay The NHS Surcharge" also when I printed the Set 'O' form they are double sided, does this matter?

Any help on this would be much appreciated .

Immigration health surcharge is NOT payable for ILR applications.

Double sided form Set(0) is fine.
Char (CR001 not Casa)
