Help V4F4A and Appendix 2 form for spouse settlement visa

My question is very simple .

There is one online application form
Which we filled up and signed by me and we sent off to home office .

But recently , I came to know
That we had to send another form filled by my wife along with documents which is called "APPENDIX 2 VAF4A"




So in total we should send two form one online application form. Which is signed by me and second suppose to fill by wife .



Is that true . Did we really made mistake ?
