ILR SET (O) Name Fields

I can't believe I actually got stuck in the Name question on the form

On the SET O ILR form questions 1.4 asks for
"Full name as in your passport"

Question 1.5 asks for
"Surname of Family Name"

I have never seen a form in my whole life that asks for First Name and Surname on one question and then asks for Surname-only on the next one.

Does 'full name' mean just the first name or both first and last names?

Question 1.4 asks for your full name as it appears in your passport, i.e. Jane Ann Doe

Question 1.5 just the surname as Doe.

This is correct and the way most HO forms are structured. No point questioning the relevance though
