Hello everyone,
I'm new to this forum and I hope I posted in the correct category.
I'm asking for a friend and would appreciate any in sight. My friend has a good job he's earning the minimum required wage and ready to start putting his application together however his wife is unable to pass the IELTS A1 test that is required before submitting the application. She lives in Pakistan and her parents claim she's always been slow in school.
Could she possibly get a letter from the Doctor explaining her situation and then become exempt from taking any sort of English test whether it be in Pakistan or the UK?

I believe she has no option but to pass the test, it is mandatory certainly for Pakistan.
She should enrol for tuition/college classes and will eventually be able to pass the test.
How many attempts has she had at the test?
Once she arrives in the UK and if she was unable to pass the A2 test after 2.5 years, she may be able to stay on the 10 year FLR(FP) route, as opposed to the usual 5 year FLR(M) route.

That's what I thought too however they were pretty adamant. She's doing the test for the 4th time tomorrow.
She is enrolled in a college but because her parents were told she won't be able to progress due to being slow they took her out of school. So she doesn't even have the basic education.
They've been married since 2015 and she's been trying since with a few breaks in between.

Yes, she has a medical report from the doctors.

This situation of your is not normal. You may or may not get solution...by throwing queries in this forum.
Rather take advice of professional Immigration Lawyer.... There will definitely be some provision for applicants with learning disability forsure.

According to HO guideline if someone is over 65 or having physically/mental condition doesn't need to fulfill the English language requirement. But neither there is any cleared guidance as how HO can be satisfied nor any member from outside UK has yet shared his experience over it. Usually within UK the 5 years route is transferred to 10 years route in that case.

Her family doctor?
An official medical report/diagnosis from experts?
I am not sure what kind of evidence would be needed to show to HO but I assume it would be extensive.
