MN1 online form Referee

I know people asking same question across forum but I can't find appropriate answer for this.

1. MN1 child need professional referee should be teacher,GP or social worker who work closely with child .
Question :
If school and GP refuse to provide reference what are the options ?

2 second reference anyone who know the child more than 3 years (over 25 and no criminal conviction )

Question:
Is this person should be British or any nationality who living in UK Is fine?

Can anyone clarify on the question above ??

I spoke to head teacher in school and she happy to provide reference but can't provide passport no (British).
Is it problem ?
