TIER 2 VISA Extension with Wife and 2 years Kid

Hi,

Our (Me, my wife and baby)'s Tier 2 expiring on 15-11-2020 completing 4 years on Tier 2 General.

I am looking forward to extend our VISAs for next 2 years, need some guideline and suggestions here.

I am currently in the same Job Code earning more than 30K.

1) What documents i required for our joint application
2) How much Bank Statement i have to maintain
3) What letters required from the employer
4) How much NHS surcharge for our application
5) When is the best and ideal time to apply
