Where to my referees write in text?

Hi people of the internet.
It is my first time doing a british citizenship application so I am quet baffeled at how strange and difficult these referee forms appear.
I have downloaded this form from the internet

and expected to have some fields to fill in. Instead these are just two pages destribing what a referee is. Pretty sure that is not the correct form to give my referees.
So in words for normal people who do not deal with this every day - where are the referee templates and is there any rule wha tto write or is it like a free text how we met and what they think about me?

Presumably you have downloaded these at the end of your online application so they should be ok.
That is the template - referees are not required to give a narrative. Note the instruction:
"This part of the form is to be filled in by the applicant’s referees once their (YOUR) photograph has been attached, as explained above. "

Referees to fill in:
- Name of applicant (YOU)
- Full Name (REFEREE)
- Signature: (REFEREE)
- Date

One of my referees shares the same house as myself but we are not related - we are just friends.
Is this a problem to the immigration?

You are not making an "immigration" application!!!

Should be fine. If HO not happy, they will ask for another referee, which will delay your application.

I used my friend who I live with as a referee and had my application approved

Hello,

I have a question regarding the 'No Seatbelt' test i took when i got caught not wearing it for the first timein my life really. I do deliveries and have gotten out of habbit.
So is this a PCN because the letter with the options do not state it as such. And do i have to declare it in my application for british citizenship if its a first time and i just did a test?
