Tier 2 visa extension documents

Hi All,

I will be submitting my tier 2 (G) visa extension of my and dependent. Can anyone confirm do I missed any documents for an extension.

• Passport
  BRP card
  Cos letter (Unrestricted)
  Tier 2 extension application form
  2 passport photos
  3 Months Payslips & P60
  Print out of healthcare surcharge NHS number
  Appointment letter (if any book appointment )

Not sure

• Maintenance fund
  Bills for proof of cohabitation
  Proof of English Test
  Contract of employment
  Qualification documents

Dependent

• Passport
  BRP card
  Copy of my Cos letter (Unrestricted)
  Tier 2 extension application form
  passport photos
  marriage certificate
  proof of address
  90 days maintenance fund of at least

I am sure this question has been asked many times but unable to find the correct answer.

2. A cos for an extension within the UK is an unrestricted cos.
