Yes - but you can provide other evidence, as described in the guidance you quoted. Do you have any concerns?- can I only provide letters from educational establishments (i.e. my Undergraduate and my PhD letters) to prove my residency? Is that sufficient?
Yes - it's the applicant who has to prove residency - those with UK ILR can use their passports because they are stamped at the border.- is the HO unable to get that information from my passport? Or are they and would that only serve as an additional evidence?
The passports of EEA citizens and their non-EEA family members are not stamped on arrival into the UK, whereas those of non-EEA citizens in the UK on routes under the Immigration Rules are.

I'm also a bit confused about the Comprehensive Sickness Insurance - what counts as one? I assume I was insured by the university all that time, wasn't I? You need to check whether this is documented. If not then would the private health insurance bought in another country (Poland) count? What if it was a travel insurance but covered "the costs of the majority of medical treatment in the UK"? As you were a full time student you will be asked about CSI (policies specifically sold for this purpose) - you need to explain what you had. I can't comment on policy requirements - though travel insurance is normally for short trips and would not cover you for five years I believe.
