Appendix 2 VAF4A Form Aug 2018 Financial Requirement Form

I've been searching for the Appendix 2 VAF4A (August 2018) Financial Requirement Form, but I keep finding either the wrong versions or ones that are behind a paywall. Does anyone know where I can download and print the correct form for free?

I’d really appreciate any help or links to the official version. Thanks in advance!

Why specifically the 2018 version?

As far as I am aware, the online form contains all the financial requirement fields.
