Visa for child born to workpermit holder

Hi I am a work permit holder under the old rule before pbs. I have a child of five month and I need to apply for her visa. Can any one inform me which application form I should use is it FLR(o) or PBS dependant? I called home office but I got two different answers one saying FLR(O) and other saying PBS Dependant.

Can anyone help me in this regard considering it as an urgency.

If you are not under the PBS, then your child may use FLR(O).
