Tier 1 Dependent application queries

Hi,

I have to apply for my dependent (2 months old) born in UK from inside the UK (as we are planning to travel). My questions are

- What is the appropriate fee? As per thisform fees are

Dependants applying seperately & over 18: £1545
Dependants applying at the same time: £1159.

She is applying separately and is under 18, so what is the fee?

- Can she travel without "VISA" and can apply from home country?
