Tier 2(General) Extension form

I am about to fill the general extension form

In the payment section Section A. If one has done his payment via the online premium service which option does he select. Or do we keep it blank and attach the premium payment receipt with the application.

keep it blank and attach the premium payment receipt with the application
Char (CR001 not Casa)
