ILR Applied n pending now looking right form for wife Applic

I have been applied for ILR Set(O) and waiting for decision. My status is Tire 1 General untill decision pending.
bit confuse now because my wife want to apply extension as dependent and currently she is my dependent. She came UK as my dependent 2 years ago.
Question is which category and which form could she use?
Could anyone help me for looking right application form.

She can continue to extend her visa on PBS Dep form before her current leave expires. She will need to pay the NHS surcharge which should be about £600 for a 3 year extension on top of the visa fee.

If she switches to FLR(M), she will lose the time she has already spent in the UK and will have to stay for another 5 years on FLR(M) before qualifying for ILR and it will be 2 X FLR(M) applications (initial and extension) and NHS surcharge payable both times.
