Cannot find form for Tier 2 extension (premium service)

Hello,

I am trying to book a Premium Service appointment to extend the Tier 2 General visa.

When i logg in (https://apply.ukba.homeoffice.gov.uk) to my account the first question is

"Which application form are you using to make your application?*"

However, there does not seem to be an appropriate form for Tier 2 General. How can i apply?

Hi,

Did this link finally allowed you to book the Premium Services appointment?

OR, did you have to fill the application and then go back to (https://apply.ukba.homeoffice.gov.uk) to book the appointment? (If so what form did you select from the drop down menu for Tier 2?)
