Affidavit for change of appearance

Hello moderators

I need to renew my daughters passport. I got her original passport when she was born now she is 5. When I checked the guidance on VFS site for passport renewal, they are asking to submit an affidavit to prove change of appearance sworn in front HCI (High Commission of India) / Notary. But I couldn't find relevant form on HCI London site. Can you advise the best way to secure an affidavit for change in appearance?

Surely an affidavit for change of appearance refers to 'cosmetic changes' and not just getting older.

For Minor Applicants ‘Change of Appearance’ affidavit is not applicable for minor applicants renewing their passports that are Under 16. All minor applicants should have Declaration for change of appearance and consent for minor applicants.
