If the HO need some information from the referee how they'll contact them?

email or telephone or post?

HO don't usually contact referees. If they do, they will use the contact information provided and likely write a letter.

Should I let them know if one of my referee has changed the address recently?

If the reference form is only being completed now ( prior to your application) then the referees should include 3 years address history, however, if it has already been done, personally, i would not bother to send a letter to them to this effect. if they really need to contact and cant get through to your referee, they would probably let you know or ask for an alternative referee.
