Anyone applying for Tier2 General from India?

Anyone who is applying to Tier2 General visa from India. And can share his/her contact detail. I see some recent changes over the English requirement so just wanted to cross check.

So here is my doubt which I am looking to clarify:

From April 2016 PBS calculator is removed ( https://www.points.homeoffice.gov.uk/gu ... lable.html ) and replaced by https://www.naric.org.uk/visas%20and%20nationality/

So to get through English requirement, what I need to do?
Can I simply pay £125+vat to UK NARIC and English requirement done by providing them my masters degree taught in English and with transcript and letter from institute saying course was taught in English?

Is there any thing called self-assessment form that I need to provide?

If currently I am in UK on dependent visa applying from India for T2 general, what address detail I need to provide as my current residency and address. Is it of India or UK?
