Spouse visa extension

Hi i am a british citizen but my wife is a Pakistani national. We got married in Sep 2013. she got her visa for 33 months in dec 2013 and travelled here to the UK within a week. Her visa expires in Sep 2016 after which she will get another visa for 2.5 years before she can apply for ILR. I have few questions that I am seeking help with;
1. She did not pay for IHS when she applied the first time as she is registered with the NHS does she need to pay IHS or not???
2. When would be the best time to apply for her visa extension???
3. I work with a British employer and have been offered a job with the same orhanization but in Saudi Arabia. If I take this oppurtunity niw how would it impact my wifes visa, as we inend to travel back and forth between Saudi Arabia and UK

1. Yes, she will have to pay the £500 immigration health surcharge.

2. Closer to expiry, but as she had a 33 month visa, you could look at starting the process about 2 months before.

3. Not sure, but to qualify for ILR, she needs UK residence. She also cannot stay in the UK if you leave. Her visa is wholly dependent on you being 'present and settled'.

Thank you for your reply CR... I was wondering if I move to Saudi Arabia after we get her visa extension, would her visa still be valid to travel to and from the UK. Also how would it effect her ILR in the future.
