How to apply for Tier 2 ICT dependent from UK

Hello,

I'm in UK on a Tier 2 ICT visa and I have applied for extension. During the visa extension application, I did not applied for dependents in the same application. Now once my new visa comes what;s the process to apply for dependent visa extension within UK ?
While filing the application form, we have to select the visa type and there is no option for "Tier 2 ICT Long term staff dependent", I can only see "Tier 2 ICT Long term staff".

Any suggestions please ?

Which form are you referring to? Online??
