Children's Visa

Hi there,
I have a quick question. I am applying for my children's visa in the Philippines. I have filled up all the application online but one thing that I was not sure is who will sign the declaration form. Because at the end of the form it says Applicant's signature, date and its printed who completed the form. My children are under 18 and they will be accompanied by my parents at the visa center. Could you please help?

Thank you! We have just sent the printed online application with 2 copies. One is signed by me and one is empty just in case.
