Unless the English test was used for ILR applied after 28 October 2013, you will need to take another test. The English requirements changed on 6 April 2015 and only those stated on the current list will be accepted.
2. I have a plan to move house, I am thinking to do that after sending application, will I be able to send HO new address later on or I am best off not move anywhere?
IMO, probably best to wait until you move to your new address until you apply.
3. Regarding referee requirement , one referee has to have a professional standing; does that mean he or she should have any profession like IT?
Have you read both Guide AN and Booklet AN. Full details of referee's can be found in there. Do read these as they provide info on who can and cannot act as a referee. Professionals that qualify can be found here.
4. Finally how to play for the application as I do not see payment fields on the form.
Again, the accompanying Guide AN and Booklet AN has the details for payment and a link of the website where you can download the payment form

Thats it. Leave question 1.23 blank. It is only for those who are using the current English test taken after 6 April 2015 as SELT unique numbers was only intorducted after that date, so you won't have one.I met the knowledge of language and life requirement to qualify for settlement on or after 28th October 2013;

I too had quite a few parkign tickets (around 3 or 4) in the year leading up to my application. I didnt declare any. My application is being processed so you may wish to wait and ill update you.
