Hi Experts
I am applying for my wife and son 4 years old
Under FLR M . As a sponsor as I hold ILR
Do I have fILL 2 separate forms or can I just do
1 form and add my sons name to it ?

Please advise

One form for both, wife as main applicant and child as dependent.

Don't forget you'll need to pay the NHS surcharge of £500 x 2

We had the PEO yesterday Morning and Al hamdulliah , Extension has been granted for wife and sin we faced no issues.
