ILR for Spouse Tier 1 G Dependent

Hi,

Can AnyOne please advise for queries below:

Recently I got my ILR (I was on Tier1-general visa) via premiun service made in Croydon. I am the primary applicant.
Now I am planning to apply my wife ILR via premium service. She has finished her 5 year continuous stay in UK.

1> Which form is the right one to use for filling. Appreciate if you can share the link.
2> What are the documents she has to carry along.

Dear Experts,

Recently I got my ILR successfully (I was on Tier1-general visa) via premiun service made in Croydon. I am the primary applicant.
Now I am planning to apply my wife ILR via premium service. She has finished her 5 year continuous stay in UK.

Questions:
In SET(O) Form,
1> Section 3- Which Category I should tick? (As there is no ILR dependent category)
2> Section 4.2 - If your partner is applying with you is he or she aged 18-64 ? (My wife only going for ILR as already I got my ILR last month)
3> Section 4.5 - IELTS SELT Consortium? (Wife Passed SELT B1 exam from Trinity College London,. I think this does not apply for my Wife!).
4> Section 9 - Does not Apply for this case

5> Do I need to provide any of my bank statements to show my income as part of this application. Currently i am doing contract work and has got
my own business account [By the way, we are providing Joint Account Statements which does not show much funds]

Please advise.

Apologies, as still I am with confusion with your above response Q 1>

I was on Tier1-General, and got ILR last month, So I think my wife might have to mention in her SET(O) form Section 3 to be on "Tier 1 dependant" catagory? [Please note i never be on tier 2]

Sorry for asking the question again. But please advise/clarify.

Then yes, Tier 1 General partner. Probably just a typo after a 3.5 hour commute home due to tube strike.

Another query
As I am working as a contractor with my own pvt ltd company since two year approx. And I am the Director holding 100% share. My wife is employee with my company and my accountant is raising invoice for minimum monthly salary amount to my wife every month. And I am not regular in transferring her salary amount into her account. Not showing her as working will reduce the complexity whilst filling the form, but hear NI number has been used for invoicing by my accountant for all months.

So Can I know, what do I need to answer in SECTION 5. I really don't prefer to show my wife's earning as she is my dependant. and it for her SET(O) application

-Section 5 - Your home and finances

5.2 Do you or your partner, or both, pay any rent or mortgage for your home?

5.3 Are you working in the UK?

5.4 Does a relative or friend of you or your partner, or both of you, regularly give you money?
