This question is about the passport form (you are filling in for JCAP I assume).eumumto2 wrote:There is a question in the passport form about my parents nationality at the time of MY birth. At that time it was USSR. Now it is Latvia.
Should I just write USSR? Or do I need to explain that this town now belongs to Latvia?
Quite confusing...
AN 1) Full name at birth.eumumto2 wrote:Hello and thanks for your help!
I have some questions.
AN form:
1) in question 1.7 - name at birth if different from above.
Should I write full name or the surname only? My first (given) name didn't change, the only Family name has been changed after marriage.
2) in question 2.4 - PR card number. Where can I find it? As I see some two numbers: one is on each page of blue card (E0123456) and the other number is on glued paper (looks like visa) and before the number is UKF 0123456. Which of them should I use?
MN1 form:
1) Should I fill section 3? Question 3.13 is about EEA national exercising EC Treaty rights. I have read the guide and still don't understand do I need to fill this question.
About us - we are a family from EU (A8 nationals). We already have the PR cards (for more than 1 year) on basis my husband was working all years that we are living in the UK. Now we decide to apply for BC only me (wife - not working) and our son 16 y.o. Husband doesn't apply for BC.
If I still need to fill this 3.13 question about all working history - I assume should I put my husband's details? I really don't understand this.
2) Who should put the signature in questions (see below) - my son 16 y.o. or myself?
- 6.21 (confirmation of wish to register the biometric information),
- 7.5 (child's consent for applying for BC),
- 8.1 (declaration - name),
- 8.10 (declaration - signature).
If your son is a minor YOU sign on his behalf. He does not sign the form.eumumto2 wrote:Thanks!
As I understand the questions 6.21 and 7.5 should be signed by my son and others by me.
You've asked this previously and been answered - in posts above.eumumto2 wrote:One more thing.
The country of my birth.
At the time of my birth it was USSR, now it is Latvia. Should I put USSR?
