No, IHS fee is only required if the visa is more than 6 months

The NHS surcharge of £600 is payable when your spouse switches to FLR(M) once you are married.

Bear in mind that there is no free access to NHS facilities while on a fiance visa and private medical insurance is recommended as any NHS treatment will be charged at 150% of the cost.
