Discretionary Leave to Remain - Further Leave - Extension

Hi,

Case 1
DLR was granted for 3 years (Letter says settlement after 6 years).
Need to apply for further leave to extend the DLR.

Case 2
Application for settlement after 6 years of DLR.

Need to know about the following for both cases mentioned above:
What form do I need to fill?
Can I use Premium Service?
Fees?

PSC is OK

What about ILR after 6 years of DLR

Can we use PSC for ILR?
