NTL Premium appointment need help

My step daughter has a premium appointment to transfer her ILE to a biometric residence card.When we booked the appointment we had to pay for the whole lot at the same time.
I am a bit confused about the payment page in the application do we still need to fill that in as the money had already been paid when the appointment was made ?.
