ICT Tier 2 visa - Options

Hi, I'm currently on ICT Tier 2 visa since Nov. 2013 and would complete 5 years in Nov. 2018 (including 2 years extension).

Except for returning back, What are my possible options? Also, are there any considerations on health related grounds (wife suffering from cancer and currently being treated).
