Tier 2 General - high earner

Hi Gurus, experts,

I am currently on Tier 2 General dependent since May 2016. I earn greater than £160k salary. I have some questions regarding a switch to Tier 2 General:
1. Can I switch to Tier 2 General from UK itself or I have to apply entry clearance from home country?
2. Is RLMT required for Tier 2 General for above high salary threshold individuals?
3. Will my current stay of Tier 2 General dependent counted for ILR?
4. Is there a faster route for ILR for high earners?

I will answer the two questions I know :
3. Will my current stay of Tier 2 General dependent counted for ILR?
No. Once you apply and are granted Tier 2 General, you start a new 5 year residence period towards ILR. You need either 5 years as dependent or 5 years as Tier 2 main visa holder, you cannot combine time on the two visas to make 5 years.
4. Is there a faster route for ILR for high earners?

No. You require 5 years residence.
