NHS Surcharge for Non-EEA

Hi,

My wife has been offered a place for Phd study (3 years) at Essex uni.

We would like to get there as family including our son, apply8ng for tier 4.

Could you please make the following questions clearer for me?
- paying NHS Surcharge is compulsory as a part of visa application?
- IELTS is required for dependents since it is written on britishcouncil page as "Life Skills A1 for spouse/partner; A2 for spouse/partner to extend after two and half year".

1. Yes, for all applicants.

2. No. As far as I am aware PBS depedents do not require the English test. You are quote a section relevant to a different visa category.

I wonder that whether we have to pay for NHS totally (3 years) at once or year by year?

You pay it upfront for the duration of the visa. If you are applying for a 3 year visa, then it is £450 each.
