I have to apply for visa extension through FLRM. It is second time I am going to apply for my wife. Can you please help me by confirming application form I need to fill. Is it the right one?

1. My passport Original and copy + my partner's passport
2. pay slips
3. Bank statements
4. Original copy of the civil partnership certificate
5. 2 passport size photos of partner and one mine.
6. Marriage certificate
7. English language test certificate
8. Biometric Residence Permit
9.Correspondence to you and your partner (4 items in joint names and 2 items addressed to each partner at the address, in total 8 items)
10. Mortgage documents/Tenancy agreements/ council tax bill

Which English test is being submitted?

£993 plus £500 health surcharge if applying by post.

You are required to submit a selection of documents from various sources covering two years to prove cohabitation. Suggest read the form, page 62/63.

Do I need to send my husband's original passport? I am his dependent, I will submit my original passport.
Do we need to send photos of both of us to show that we are still together?
We have two bills in my name and my husband's name are they enough? and we have 2 each proof of address individually.
