FLR(M) switch from Tier 2 dependent cohabitation documents

Hi.

I have been on tier 4 then DES and now on tier 2 visa. I am planning to apply for my ILR through the same day service.

Alongside, I am planning to apply for my wife and baby through FLRM.

I have few questions:

1. Only one form needed for both wife and baby?
2. About cohabitation documents, do i need those 6 sets (as she is switching from Tier 2 dependent to FLRM and hence this is a fresh application for FLRM not the renewal), do i still need to show these.
3. Under children section it says something like this
Evidence that your parent plays an active role in your upbringing.
Does this apply to her application too

On what basis are you applying for ILR, 5 years Tier 2 or 10 years long residence?

2. Yes, you need to provide examples of what the form says on page 63/64.

Thank you for the reply. The child was born in UK.
My question 3 is related to page 65, as below, do i need to provide all this?

Evidence of where your child/children normally lives and that you and/or any partner play an
active role in their upbringing. This evidence should be dated within the last 3 months and
can include:

Indeed a Good question. I was intending to do his passport straightaway after my ILR. But I found out that MN1 application can take anywhere upto 6 months, but I want to be able to travel soon with him and my plan is as follows:

I will apply for my child's tier 2 dependent visa through the same day service and travel with him and when i do come back i will apply for my ILR and FLRM for my wife and MN1 for my child. Apologies for the question 3, you can ignore that as that is not in my plan it was planned earlier.
