TIER 2 change of employer

Dear all,
Could you please help me in the following situation.
I am currently on my first Tier 2 general visa which was for 3 years with 3 dependants , but currently i was accepted by another employer to start a new job in which he will provide me with a new COS for 1 year.
My questions are:
1. I will apply for an extension on my tier 2 to change employer, should I include my dependants in the application as they still have 2 more years on their Tier 2 dependent visa?.or they just can keep that without a new application?.
2. During my first application i paid a health surcharge for 3 years for all of us (and one year only have passed since then), can i use that reference number in the new application ? , or should i pay for a new one?.
3. If I to apply for all at the same time , can I choose the same (premium )day visa option just for my application ( to avoid paying for every one), and keep my dependants on the standard visa pathway?, or it is a must to apply on the same pathway for the dependants?.
4. what documents do i need to provide for me and my dependants?.
5.My notice period for my current employer is 3 months, and I understand that the visa could take up to 8 weeks in the standard pathway , but my new job should start not beyond 3 to 3 and a half months from now, so should I wait for my visa to change first, before giving my notice ?( which might take 4 to 5 months),or give the notice now and hope for the best?.

1. No need to include dependents.

2. No you can't. You must pay IHS again.

3. Yes you can. Your dependents do not have to apply for a visa if they still have a valid visa for 2 years.

4. You do not need to apply for dependents.

5. Wait till you have your new visa before you resign. You will need to negotiate with your current employer.

Thank you very much for your help CR001.
i was confused because in the GOV.UK website they say that you should include your dependants in your extension, but basically it is not an extension. as their visa is still valid, unfortunately, the pathway to change employer is through visa extension!!!!!!!!!!!!!.hence the confusion.

Apologise for the previous post, it was done by mistake.
Could you please clarify on the notice period serving.
as i will be issued a new BRP for the new employer, and then i will serve my notice period.
is it legal or is it OK to work for the current employer, to serve the notice time, with a new BRP that is assigned to a new employer? although the new BRP will be issued as soon as the application is approved, which could mean working for the current employer for 2 months withe the new BRP?.
and if i can not do work after the new BRP is issued, what is my options?, should i give the notice period first the apply for the VISA in the last 2 weeks via One day service for example?.

Dear All;
help and clarification is appreciated for the following.
I am Currently on A Tier 2 valid for 2020, but i have a new job offer starting at the 15/11/2017 and a new COS will be issued this month with that start date 15/11. and i have given the 3 months notice period for my current employer so i could start my new job in time, otherwise i will lose the job for any delays beyond the above date.
my question are.
1. when the new COS is issued, will i be legally allowed to work for my current employer, as i believe the COS will specify the start date in 15/11.? if yes the please see number 2.
2. if i apply for the BRP immediately after getting the COS, will it be assigned withe same start date of 15/11? or to the date of application approval?,and will i be legally allowed to work for my current employer withe new BRP although i will be still serving my notice period?.and the COS date is 15/11?.
I had contacted Home office with no clear answers and they confused me about the rules.
