EEA PR or EEA FM?

Hello, I am writing to you because I got confused which form I should use to apply for my husband's Permanent Residence Card. His visa: Residence Card of a Family Member of an EEA National expires in February 2018. Till now I thought I have to use EEA PR form to apply for his PR, but today I found one more form EEA FM. I am really confused which one I should use as they look almost the same.
