Advise required for ILR Dependent application

All Gurus in this forum,
i hope this sounds silly, after looking atleast few questions in this forum, i still not have an answer for my question and thought to drop down my first query in this forum.
I'm a tier 1 dependent and i have booked PEO appointment on 13th Sep 2017.
I'm applying my own as my spouse is already British citizen after naturalization.

1. What do i select in Section 12

2. Documents which i have prepared is:

• Set(o) form
  My passport and visa - original and copy
  Spouse British passport - copy
  Marriage certificate - original and copy
  Letter from accountant showing my salary and dividend for 2016-2017 & 2017 - till date
  My Past 6 months UK bank statements (Joint Account) show employment and maintenance funds in the last 3 months - original and copy
  My Past 3 months payslips - printouts
  Water bill showing my name and address
  Latest council tax bills (with spouse name) - original and copy
  HMRC letter my name on it- original and copy

let me know if i have missed anything.

Ok, given that there are 4 usernames posting form the IP address, I will monitor the posts for a few days.

That will be correct as i tied to create login yesterday and today from 3 different email id i had and i never received the activation email. it was only this email id i received a activation email.

is it possible to get a response for my questions please.
