Switch from Tier 2 ICT issued in March 2011, to Tier 2 General

I have two questions regarding my UK visa

- I am on Tier 2 ICT issued in Feb/March 2011. I understand I can extend my visa till 10 year if current employer continue to sponsor. If I get sponsor from new employer in UK then can I switch to Tier 2 General within UK or I need to leave my current employer & apply from outside i.e. from my home country?

- My wife is on Tier 2 ICT dependent visa. If she get sponsorship from her employer then will she have to apply from outside UK for Tier 2 General or can she apply from inside UK?
