Tire 2 to ILR Dates

Hi CR, Please help advise regarding the dates to apply for ILR

Visa Type: Tier 2 General (Restricted Cos)
COS letter dates: Startdate 30/10/14 and Enddate - 30/10/19 and assigned on 11/11/2014
Visa Valid From: 04/12/14 and Valid Until: 30/11/19
Entered in UK on 12/12/2014
