T1E Extension Application

Hi Forum Members,

This one might be a silly question.
I would like to know when applying for a T1E extension application for myself, do I have to apply for my defendants (spouse and child) using the following form:

or is there an online application form option as well?

This might be another silly question, however, while going through the T1E extesion application for the main applicant. it says as shown in the screenshot below:

Its a bit confusing, whether I can make the payment using the HSBC debit card (VISA, not DELTA or MASTERO) ?

Application form Page 4, Heading "How you can pay" says:

Application form - Page 4 - How you can Pay.
